Monthly Progress Report

Monthly report to be submitted on the progress of the workers activity with the families child.

SSAH & ACSD Support Workers

Families who receive funding through the Special Services at Home (SSAH) program or Assistance for Children with Severe Disabilities (ACSD) program often wish to hire an in home worker for their child.  The worker contracts with the family to provide regular hours of service.  During this time the worker spends time with the child to develop skills, build relationships, participate in the community and work on specific goals.

Special Services at Home Timesheet

Use this timesheet to submit hours applied against your SSAH funding.

Special Services at Home Family Invoice

Family invoices are to be submitted by the 10th of each month. Receipts must be attached to invoice for payment. Invoices must be submitted within 30 days of deceipt date.

Special Services at Home Application

The SSAH program is available for children and adults with a developmental disability and children with a physical disability who are residents of Ontario. SSAH can help individuals with disabilities to live at home with their families by providing time-limited funding to purchase supports and services not available elsewhere in the community.

The application contains mailing instructions, application form, and progress note.

Special Services at Home

The Special Services at Home Program is a family directed program funded provincially by the Ministry of Community and Social Services. The Program is available for children and adults with a developmental disability and children with a physical disability who are residents of Ontario.
The aim of the program is to make it easier for individuals with disabilities to live at home with their families.
